Welcome to the Frequency Lifestyles Menopause Education & Empowerment Retreat.


This retreat is designed for women navigating perimenopause, menopause, and post-menopause through education, nervous system support, movement, and community connection.


This document outlines your acknowledgment of potential risks, your health declaration, and your agreement to release Frequency Lifestyles Pty Ltd (the Company) from liability related to your participation in the retreat.


Participation in the Menopause Education & Empowerment Retreat is strictly limited to individuals aged 18 years and over. No persons under the age of 18 will be permitted to attend or participate in any retreat activities, workshops, or accommodation arrangements. The Company reserves the right to verify age and refuse entry if this condition is not met.

By signing this form, you acknowledge the following:


1. RISKS AND RESPONSIBILITIES

Your participation in this retreat is entirely voluntary, and you accept full responsibility for your health, safety, and wellbeing throughout all activities.


You acknowledge that the practices included in this retreat may not be suitable for everyone and that they carry inherent physical, emotional, and psychological risks. By participating, you willingly assume all risks associated with these activities.


If at any time you experience discomfort, pain, dizziness, emotional distress, or any other adverse reaction, you agree to stop, modify, or rest as needed and to seek guidance or support from the instructor or facilitator.


You agree to:

  • Follow all instructions and safety guidance provided by the facilitators or instructors.

  • Accept that failure to comply with such guidance is at your own risk.

  • Take full responsibility for any injury, illness, or other effects arising from your participation.

  • Be solely responsible for the security and safekeeping of your personal belongings. The retreat organisers are not liable for lost, stolen, or damaged items.


Activities at the retreat may include, but are not limited to:

  • Educational workshops and guest speaker presentations

  • Breathwork sessions

  • Laughter yoga

  • Yoga and gentle movement practices

  • Nervous system regulation exercises

  • Meditation, mindfulness, and relaxation techniques

  • Group discussions and emotional processing sessions

  • Use of retreat venue facilities and grounds


Participation may involve, without limitation:

  • Physical exertion or stretching

  • Altered breathing patterns and increased oxygen intake

  • Emotional expression or release

  • Nervous system activation, regulation, or fatigue

  • Temporary sensations such as dizziness, tingling, light‑headedness, muscle tension, or fatigue


By signing this waiver, you acknowledge that you fully understand and accept the above risks and responsibilities and agree to participate with awareness and care for your personal wellbeing.


2. PERSONAL RESPONSIBILITY AND MEDICAL DISCLAIMER

You acknowledge that all information, practices, and discussions offered during this retreat are for educational and general wellness purposes only. Guest speakers and facilitators may share knowledge, techniques, or experiences but will not provide individual medical, psychological, or financial advice unless explicitly qualified and engaged to do so.


You understand that participation in this retreat is not a substitute for medical care or mental health treatment. You are responsible for consulting a qualified healthcare provider regarding any existing or emerging health concerns before, during, or after the retreat.


You confirm that you have accurately disclosed all relevant health conditions or concerns to the organisers, instructor, or facilitator that may impact your ability to safely participate. You agree to update them if your health status changes before or during the retreat.


You acknowledge that it is your responsibility to inform the Company in advance of any dietary requirements, food allergies, or intolerances, and that while reasonable efforts will be made to accommodate these needs, the Company cannot guarantee an allergen‑free environment.


By signing this agreement, you accept full personal responsibility for your wellbeing and acknowledge that your participation is based on your own informed decision and self‑awareness.


3. LIABILITY WAIVER AND RELEASE

By signing this form, you acknowledge and accept that your participation in the retreat is entirely voluntary and at your own risk.


You irrevocably release, waive, and discharge the Company, its directors, authorised facilitators, guest speakers, employees, contractors, volunteers, and representatives (collectively, the “Released Parties”) from any and all claims, demands, damages, or causes of action that you have now or may in the future have arising out of or in connection with your participation in the retreat.


You agree to assume full responsibility for all known and unknown risks, including physical, emotional, and mental effects that may result from participating in any retreat activity.


You further agree that the Released Parties will not be held liable for any loss, injury, damage, or claim — including those resulting from negligence — whether occurring before, during, or after the retreat.


This release applies to all forms of participation, including workshops, movement practices, breathwork sessions, group discussions, and use of the retreat venue or facilities.


4. CONFIDENTIALITY AND CONSENT

You understand that your personal information will be kept confidential and not shared without your consent and any recordings or images taken during sessions will only be used with your permission.


Your personal information (including sensitive health details provided above) will be handled in accordance with the Company’s Privacy Policy, available at https://www.frequencylifestyles.com.au/copy-of-copyright-notice or on request. This includes how we collect, store, use, disclose, and protect your information, and your rights to access or correct it.


We may take photographs or videos during the retreat for promotional purposes.


If you do not wish to be photographed, please notify us in writing prior to the event.


5. CODE OF CONDUCT

All participants must adhere to the following standards of behaviour throughout the Retreat:

  • Treat all participants, facilitators, guest speakers, staff, and venue personnel with respect. No harassment, discrimination, bullying, or aggressive behaviour of any kind will be tolerated.

  • Maintain strict confidentiality regarding personal stories, health disclosures, or sensitive information shared by others during group discussions, workshops, or activities.

  • Arrive and participate free from the influence of alcohol, recreational drugs, or any substances that may impair judgement, coordination, or emotional regulation.

  • Engage constructively in sessions without dominating discussions, using electronic devices disruptively, or interrupting facilitators or other participants.

  • Follow all venue rules, fire safety protocols, and directions from staff regarding facility use, parking, noise levels, and quiet hours.

  • Immediately inform facilitators of any health changes, injuries, or conditions that arise during the Retreat which may affect your participation or others' safety.


The Company reserves the absolute right to immediately remove any participant who breaches this Code of Conduct, without warning or refund. The Company may also ban the individual from future events and pursue any legal remedies for damage, injury, or disruption caused.

Participants removed for conduct violations forfeit all fees paid and any accommodation arrangements.


6. ACCOMMODATION AND VENUE

Participants acknowledge and agree to the following terms regarding accommodation and venue use:

  • Room assignments are at the Company’s sole discretion and subject to availability. The Company reserves the right to change room allocations at any time. Participants must not enter restricted areas or other participants' rooms without express permission.

  • Participants are fully liable for any damage to venue property, furnishings, equipment, or other participants' belongings caused by themselves, their guests, or persons under their control. Participants agree to pay for repairs or replacement costs upon demand.

  • The Company accepts no responsibility for loss, theft, or damage to personal property (including valuables, luggage, electronics, or medications) under any circumstances. Participants are strongly advised to use venue safes where provided and avoid bringing irreplaceable items.

  • Respect designated quiet hours and keep noise to a minimum at all times. Excessive noise or disturbances may result in immediate eviction without refund.

  • Leave accommodation areas in reasonable condition upon departure. Remove personal rubbish and strip beds as directed. Additional cleaning fees may apply for excessive mess.

  • Comply with specified check-in and check-out times. Late check-out or early check-in is not guaranteed and may incur additional charges.

  • No smoking, vaping, or use of recreational substances permitted anywhere on venue premises, including outdoor areas. Designated areas (if any) must be used.

  • No external guests permitted in participant accommodation or common areas without prior Company approval.


Violation of these terms may result in immediate removal from the Retreat and venue without refund, plus liability for any costs incurred by the Company or venue owner. The Company reserves all rights to pursue legal remedies for damage or losses.


7. TRAVEL INSURANCE

We strongly recommend that all participants obtain comprehensive travel insurance prior to booking, covering trip cancellation/curtailment, medical emergencies, hospitalisation, repatriation, travel delays, baggage loss, pre-existing medical conditions (including menopause-related issues), and personal liability. The Company accepts no responsibility for any travel-related costs including flights, accommodation, transfers, visas, expenses from delays or cancellations, or medical evacuation/treatment. Proof of coverage may be required upon request. Without adequate insurance, participants bear full financial risk for all cancellation, medical, and travel-related expenses.


8. FORCE MAJEURE

The Company is not liable for any cancellation or changes due to circumstances beyond our control, including but not limited to natural disasters, government restriction or venue closures.


9. REFUND AND CANCELLATION POLICY

At Frequency Lifestyles Pty Ltd, we value the time and energy our facilitators dedicate to creating high-quality workshops, events, and sessions. To ensure fairness and clarity for all participants, the following refund and cancellation terms apply.


DEPOSITS

Due to venue requirements and event-planning costs, the following applies unless otherwise stated:

  • Deposits are strictly non-refundable under any circumstances.

  • Deposits may be transferrable to another participant with the Company’s prior written approval.

  • By paying a deposit, you acknowledge your understanding and acceptance of these terms and your commitment to the full retreat booking.


WORKSHOPS & EVENTS

  • Cancellations made more than 60 days before the retreat will be issued a full refund excluding the initial deposit.

  • Cancellations made between 30 to 60 days prior to the retreat will be issued a 50% refund excluding the initial deposit.

  • Cancellations made within 30 days will not be issued a refund.

  • If you are unable to attend, you may transfer your booking to another participant subject to the Company’s approval.


PRIVATE SESSIONS

  • A minimum of 24 hours’ notice is required to reschedule or cancel a private session.

  • Cancellations made with less than 24 hours’ notice, or “no-shows,” are non-refundable and non-transferable.

  • Some retreats or special sessions may have their own specific terms, which will be provided at the time of booking.


EVENT CANCELLATION BY THE COMPANY

  • The Company reserves the right to cancel, postpone, or reschedule any event, session, or class at its sole discretion.

  • Facilitators, instructors, or locations may change without notice.

  • If the Company cancels or reschedules an event, participants will be offered either:

    • A full refund, or

    • An account credit redeemable for a future booking.

  • The Company is not liable for any travel, accommodation, or incidental costs incurred due to event changes or cancellations.


EXCEPTIONAL CIRCUMSTANCES

In compassionate cases such as serious illness or emergency, requests for refunds outside the above terms may be considered at the discretion of the Company. Supporting documentation (e.g., a medical certificate) may be required to process such requests.


PAYMENT PLANS

If you choose to pay for your retreat booking via the Wix third‑party payment platform, you acknowledge and agree that:

  • You remain liable for the retreat cost, regardless of cancellation or non‑attendance, in accordance with this clause.

  • Any cancellation of your retreat booking does not cancel your payment obligations with the third‑party provider.

  • The Company is not, and will not be responsible for fees, interest, or cancellation charges applied by the third‑party provider to you.

  • By proceeding with a payment plan, you confirm that you understand and accept these conditions.

By signing below, you confirm that you have read, understood and agree to the terms and conditions outlined in this waiver.

I am participating in the Menopause Education & Empowerment Retreat with Frequency Lifestyles Pty Ltd. I accept responsibility for informing Frequency Lifestyles Pty Ltd its authorised facilitators, guest speakers, volunteers, employees, and/or representatives about any medical conditions, injuries or changes to my health that may affect my practice, prior to a class commencing. I understand it is my personal responsibility to consult with my doctor or health practitioner regarding my participation. I have no medical or mental condition, which would prevent me from taking part in the retreat. If my health condition should change, I agree to fill out a new Waiver of Liability form and advise Frequency Lifestyles Pty Ltd of the new changes. I understand the instructions are intended as guidance and I assume responsibility to practice within my personal limits and to decide whether or not to follow the suggestions provided by the instructor. I have read the above release and waiver of liability and understand its contents. I agree to the terms and conditions stated above.
